I understand that if my child or children have more than 3 unexcused absences they will not receive their First Holy Communion

Disruptive Students with 3 or more warnings will not be allowed to receive their First Communion. Parents are not to enter any of the Classrooms at any time. Parents must check in with the main office.

Child(ren) must be dropped off before 1:50 PM and picked up no earlier than 4:45 PM and no later than 5:00 PM. St. Peter Volunteers will not be responsible for children after 5:00 PM.

I understand that reasonable precautions will be taken to safeguard the health and wellbeing of the participants in this Catechism Class and that I will be notified as soon as possible in the event of an emergency. In the case of sickness or an accident, I authorize and consent the Catechism Team, or other associated volunteers of the Catechism program to obtain medical care from a licensed physician, hospital, or medical clinic for my child in the event that myself or other legal guardian(s) cannot be reached. I hereby do release and forever discharge St Peters Chaldean Catholic Cathedral, The Chaldean Catholic Diocese of St Peter the Apostle, its officers, clergy, directors, board members, and volunteers from all manners of actions, claims, which I or the child(ren) named above shall or may have for any reason, arising during my child’s attendance of the Catechism Class. Unless other written instruction is submitted, I also consent to allowing my child’s image to be recorded, either by photograph or video, and used during all the Catechism Classes and the day of and after the First Communion ceremony online or in print.
