Youth Convention 2025 Registration

I agree to observe and obey all posted rules and warnings, and further agree to follow any oral instructions or directions given by representatives of St. Peter's Chaldean Catholic Eparchy, its clergy, its volunteers and youth leaders, or the employees, representatives or agents of St. Peter's Chaldean Catholic Eparchy.

I recognize that there are certain inherent risks associated with the above described activity and I assume full responsibility for personal injury to myself and (if applicable) my family members, and further release and discharge St. Peter's Chaldean Catholic Eparchy for injury, loss or damage arising out of my or my family's use of or presence upon the facilities of St. Peter's Chaldean Catholic Diocese, whether caused by the fault of myself, my family, St. Peter's Chaldean Catholic Eparchy or other third parties.

I agree to indemnify and defend SSt. Peter's Chaldean Catholic Eparchy against all claims, causes of action, damages, judgments, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from my or my family's use of or presence upon the facilities of St. Peter's Chaldean Catholic Eparchy. Any legal or equitable claim that may arise from participation in the above shall be resolved under California law.

I understand that reasonable precautions will be taken to safeguard the health and wellbeing of the participants in this Retreat and that I will be notified as soon as possible in the event of an emergency. In the case of sickness or an accident, I authorize and consent the Youth Convention Team, or other associated volunteers of the Retreat program to obtain medical care from a licensed physician, hospital, or medical clinic for my child in the event that myself or other legal guardian(s) cannot be reached. I hereby do release and forever discharge St Peters Chaldean Catholic Cathedral, The Chaldean Catholic Diocese of St Peter the Apostle, its officers, directors, board members, and volunteers from all manners of actions, claims, which I or the child(ren) named above shall or may have for any reason, arising during my child’s attendance and participation of the Retreat. Unless other written instructions are submitted, I also consent to allowing my child’s image to be recorded, either by photograph or video, and used during all the Retreat.

I agree and acknowledge that I am under no pressure or duress to sign this Agreement and that I have been given a reasonable opportunity to review it before signing. I further agree and acknowledge that I am free to have my own legal counsel review this Agreement if I so desire. I further agree and acknowledge that St. Peter's Chaldean Catholic Diocese has offered to refund any fees I have paid to use its facilities if I choose not to sign this Agreement.

I consent that St. Peter's Chaldean Catholic Eparchy can communicate with me via mobile phone, messages, e-mail and any kind of online communications.
